Transitional Care Team
The Transitional Care Center at Elmhurst Memorial Hospital uses a team approach to provide quality, personalized care. Patients, their families, physicians, nurses, social workers, therapists, registered dietitians and pastoral care staff work together, focusing on each patient as a whole. During their stay, patients remain under the care of their personal physician. The team’s objective is to assist the patient throughout the recovery process so that the patient can return to an independent lifestyle.
